Rollyson Brazilian Jiu Jitsu

Waiver of Liability: Rollyson Brazilian Jiu Jitsu, LLC, hereafter referred to as Rollyson BJJ, urges all students to obtain a physical examination from a doctor before participating in any exercise program or using any exercise equipment. By signing this waiver, I affirm that I and/or my minor children are in good physical condition and do not suffer from any known disability or conditions that would prevent or limit my participation in this exercise program. I agree that by my, and/or my minor children’s, voluntary participation in this program, the use of equipment and facilities of Rollyson BJJ shall be undertaken at my and/or my minor children's sole risk. I hereby expressly forever release and discharge Rollyson BJJ, its agents, affiliates, their respective administrators, directors, coaches, employees, other participants sponsoring agents, volunteers, property owner(s), and owner of premises of any claim, injuries, damages, actions or cause of actions and from all acts of negligence, active or passive and all other fault on the part of those listed above or property arising out of or connected to premises where Rollyson BJJ is located whatsoever; from (a) the club’s negligence, (b) any other student’s, member’s, guest’s or invitee’s conduct, (c) the conditions of the club’s facilities, (d) student’s use of club’s facilities and activities, including but without limitation to students and/or my minor children’s use of the Rollyson BJJ’s parking lot, athletic facilities, athletic equipment, showers or any facilities and activities associated with Rollyson BJJ. Jiu Jitsu is a full-contact sport, and I fully understand that I and/or my minor children will be training in close proximity and contact with other individuals, both male and female. I fully understand that I and/or my minor children can incur serious bodily injury, either temporarily or permanently. Conditions may include but are not limited to heart attack, musculature and or skeletal injuries, dehydration, injuries to all or any joints, limbs, and face, or any other illness, soreness, or injury that I and/or my minor children may incur, including disfigurement or death. I and/or my minor children assume all risks of injuries and responsibilities associated with using Rollyson Brazilian Jiu Jitsu athletic facilities, including any athletic activities, rooms, or other club usage. The undersigned assumes all the risks inherent and incidental to this type of sports activity as conditions for applying for admission to Rollyson Brazilian Jiu Jitsu and hereby assumes sole responsibility for any and all medical bills incurred in the event of an emergency. Applicable for all Law Enforcement Agents; I hereby affirm that all techniques taught by Rollyson Brazilian Jiu Jitsu, its agents, affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agents, or volunteers, does not supersede department policy. Rollyson Brazilian Jiu Jitsu reserves the right to dismiss any student at any time for misconduct or actions that may convey a bad image of this school. The student agrees to refrain from using the knowledge obtained from Rollyson Brazilian Jiu Jitsu, with or without compensation, except for that student’s own personal growth and development. A student must not work in or own a martial arts school within 20 miles, in all directions, from any Rollyson BJJ. I agree and consent that Rollyson Brazilian Jiu Jitsu LLC. and its nominees and assigns may use the likeness and/or photography taken of myself and/or my minor child(ren) or any reproduction thereof in any form, style, or color together with any writing and other advertising or publicity material, including television and internet, in connection therewith, including the use of my and/or child(ren)’s name in perpetuity, as they may select. The consent and release are given for valuable consideration and without limitation upon, or liability for, any use for advertising, illustration, publication, or broadcast of every kind, or in a trade or media or for any purpose. I further agree that such photography and/or likeness, and the film, tape, plates, and negatives thereof, shall be and remain the exclusive property of Rollyson BJJ. I waive any right to inspect or approve the commercial or advertising material. By signing this waiver, I certify and affirm that I have read and understood this agreement and its terms and conditions, will abide by the rules and regulations of Rollyson BJJ, and agree to all terms and conditions. If the student is under the age of 18, consent in the form of a signature below must be given by a parent or guardian. By signing this waiver, I hereby affirm I am the legal guardian of the minor person referred to above, and I agree and consent to the foregoing in such capacity, as well as individually.
